This form is for Case Faculty or Staff who would like to modify their Sprint PCS order.
- If you placed your order less than 24 hours ago: You may change any part of your order, including the phone type, by completing this form. You may also cancel it entirely at no charge by calling the Telephone Services Office at 368-3900 within 24 business hours of placing your order.
- If it has been more than 24 hours since you placed your order: You may modify your service plan or extra features by completing this form, but you cannot change the phone type.
- If you need to cancel your order entirely after 24 hours: Please call the Telephone Services Office at 368-3900. An early termination fee will apply.
- Employees who have already received their Sprint PCS phones: You may still change your service plan or extra features at any time by completing this form.
